Portable Ops draws heavily from Metal Gear Solid 3: Snake Eater , utilizing the camera system from the Subsistence edition of the game. The squad is then sent into battle. During missions, the player controls only one squad member at a time. A variety of methods can be employed to expand one's squad. In addition to the standard male characters, the player can recruit and control women scientists and officers as well. Characters who are killed in combat are eliminated from the player's squad permanently. Another new feature is the surround indicator added to the game's HUD.

It's implied right there in the title, after all. In reality, though, Portable Ops Plus is more of a companion to the original; it features the same fully 3D, stealth-action gameplay , but ditches the story mode in favor of a ton of improvements to the game's multiplayer modes. In place of a story is the new "Infinity Mission" mode, which puts players through a set number of short, randomly generated missions set in partial levels recycled from the first Portable Ops. Although these still put you in control of four-man "sneaking teams" which you'll control one at a time, while the rest hide in boxes , most of them just revolve around finding an exit while evading or killing, or recruiting guards. A few will require you to take down every enemy in the level, stay alive through a timed alert or try and find the exit while sneaking through a ton of automated traps, but in general they're pretty cut and dry.

The original game's Story Mode is replaced by a new Infinity Mission mode that's set shortly after the events of San Hieronymo Incident. The player's squad now have an increased capacity for up to soldiers, allowing for more options such as roaming through Infinity Mission mode or fighting online using captured soldiers. Portable Ops Plus was released in Japan in two editions: by itself and as part of a deluxe package that also includes the original Portable Ops. While the original Portable Ops is not required to play Portable Ops Plus , save data from the original can be transferred over to the expansion. The story mode within Portable Ops Plus has been removed, and a new single player mode has been added entitled the Infinity Mission, where the player must go through randomly chosen maps, looking for the advance point within the level, using the surround indicator to help them.
